Figuring Out Health Care Administration Challenges Makes Public Health Goals Realizable for American Communities: Insights of A Public Health Advocate

Healthcare in the U.S. revolves around policy, planning, programming, health reform initiatives and innovations. Here in also lies the challenges Healthcare administrators face.  The U.S. has seen 258% increase in total contract labor expenses for hospitals in 2022 compared to 2019 (Milligan, 2023).

The challenges vary due to geo-location, standard of living, political, social, physical, cultural, economic status, gender, civil, structural and bio-medical enabling and responsive contexts. This is manifested in the nature of reforms, the healthcare system, financial uncertainty, value-based purchasing, technology implementation, patient access, care coordination and provider-payer relationships.

The enabling and responsive contexts influence social, structural and financial requirements which in turn affect the way consumers (health care seekers), providers and systems demand or engage in the uptake of health care services. This gives the backdrop against which health care managers and administrators must navigate to contribute contexts fostering better health outcomes for all. The Affordable Care Act resulted in an increase in health seekers and providers serving in a range of Health Care Institutions. This was coupled with State mandates which included establishing streamlined reimbursements, licensure of facilities and staff, ensuring proper capital expenditure records and timely meeting of mandated health benefits all of which affected service demand and provision (Buchbinder et al, 2021). 

Recent reforms in reimbursement models and moving from fee-for-service to value-based care, led to a resultant financial uncertainty for healthcare institutions following these changes. Between 2019-2022, Hospital-based expenses (labor, wear, tear, maintenance and depreciation costs) increased by 17.5%, outpacing Medicare reimbursement, which had increased 7.5% during the same time. In the same period, labor costs, increased 20.8%. The reforms affect leadership, management and performance effectiveness within organizations (Azad et al, 2017). Performance that leads to better health outcomes, comes about as a result of grounded emotional intelligence, intra-personal and interpersonal skills, possession of a range of critical skills such as self-awareness, self-regulation, self-motivation, empathy, and social skills. It is these skills that enable one to adapt to changing situations, engage in intentional problem-posing and solving, employ empathy and create a motivating workplace for the workforce to contribute to the mission of institutions (Buchbinder et al, 2021).

The novel reimbursement model includes contracting staffing agencies to recruit, onboard and fill up workforce gaps. This is in anticipation of meeting institutional and health seeker demands. Trends in health reforms now include the adoption of Digital Systems, Health Information Technology and the use of Electronic Health Records (EHR). This means that Health Care Institutions must set aside time to modernise their virtual office platforms, take staff through trainings in order to be compliant and be able to implement the new technologies.

The modern health care facility or practice has to invest in expensive Information systems management, new medical equipment and devices to comply with policy, programming and planning that requires standardized data collection and compliance through use of modern medical equipment (Buchbinder et al, 2021).

The medical devices and equipment or care products range from what are classified as implantable, life-saving, life-sustaining devices, ambulatory monitors, audiometers, bone densitometers, EKG & ultrasound machines, scales, examination room tables, cabinetry to heavy-duty hardware such as ultrasound machine. Health facilities with modern and advanced devices and equipment have reduced on the waiting-time, improve access to care, establish better coordination, serve faster, more efficiently, increase volume of health seekers and are able to generate more revenue. It is in these contexts that improved quality of care and reduced mortality rates can be realized (Sindhu, 2023).

Modernising healthcare systems means to address the different enabling and responsive contexts. This is what will make the Health Care Systems and Structures in the U.S., contribute to netter health outcomes for all.
