Overview:

Social Workers work as part of an interdisciplinary team using a family-cantered approach to support children and families to identify and achieve their goals. Social Workers aim to empower families to live their best lives, to harness their own internal strengths and those of their extended families and communities.

 

Duties / Responsibilities:

·        To promote the National Outcomes for Children and their Families

·        To work in partnership with children and young people, parents, families/carers and others as appropriate to address the needs of those who use the Service.  

·        To work alongside the Children and young people with Complex Disability towards the achievement of their maximum potential and participation, and to ensure the person enjoys the best quality of life possible.

·        To work specifically with the needs and goals of the person in the context of their relationship system – family, carers, day and residential staff, community - exploring and addressing whatever in the person’s life is helping them to make progress and what if anything is impeding their progress and contribution.

·        This involves a comprehensive assessment of Children and young people with Complex Disability and his/her family and where necessary ongoing social work and periodic reviews.

·        To identify the most appropriate social work methods that would best enhance the maximum functioning of the Children and young people with Complex Disability.

·        To maintain the person’s and family’s right to relationship of mutual trust, privacy, confidentiality and responsible use of information.

·        To be responsible for linking clients with available resources whether these lie within the client themselves, their family, the service or their wider community.

·        To inform people those we support and their families of their rights, entitlements, benefits and services.

·        To advocate for the rights and needs of people with a disability and their families and to liaise with management and other agencies as appropriate.

·        To work towards remedying deficits in service provision where identified.

·        To participate in client focused meetings, reviews and other relevant meetings where appropriate.

·        To participate in the delivery of training and group work opportunities for service users, families, staff and external groups.

·        To maintain and promote good communication and relationships within the social work team, Network team and relevant others across the Service and work within agreed policies and codes of practice.

·        To participle positively in the development of a professional social work team within the service and at Network level.

·        To maintain a high standard of documentation and providing relevant statistical information.

·        To engage in appropriate research projects and the resulting publications as requested by the CDNM / Social Work Manager.

·        To engage in social work supervision and participate in the Service’s annual development review system.

 

·        To be familiar and comply with health and safety procedures.

·        To maintain a high standard of work performance, attendance, appearance and punctuality at all times.

·        To advise management on emerging issues in social work relating to Children and young people with Complex Disability and their families.

·        To promote the work of social workers to ensure that staff are aware of the supports available from the social work department.

·        To display a positive attitude, flexibility, resourcefulness and creativity.

·        Develop and nurture a positive team spirit within the CDNT and social work department.

·        To Promote integration with the wider community as appropriate to the needs of the individuals and to contribute to a greater understanding of the needs of Children and young people with Complex Disability and their families, to their right for equality and to anti discriminatory practices within society.

·        To maintain awareness and act in accordance with all legislation relevant to the field of disability and to the protection and the welfare of the Children and young people with Complex Disability.

 

ADDITIONAL TASKS AS SOCIAL WORK TEAM LEADER

  • To support the CDNM in the development of the Children’s Disability Network Service.
  • On behalf of the CDNM support, supervise and manage the Social Work team and service within the Network, and across the wider Service as required, in coordination and consultation with the CDNM and the Social Work Manager.
  • Develop and maintain high standards of clinical practice and an effective social work service in cooperation and collaboration with the Social Work Manager, in consultation with the CDNM and in response to the needs of children, families and the Service.
  • Ensure CORU registration and CPD are maintained for self and supervisees.
  • The Social Work Team Leader will be expected to lead and motivate in order to optimize service delivery by developing teams and promoting change management initiatives.

·        Demonstrate initiative and innovation in identifying areas for service development and improvement and take the social work lead in key areas of service development.

  • Contribute to the ongoing development and implementation of quality assurance for the department.
  • To maintain a high standard in relation to assessment, diagnostic, therapeutic and recording procedures.
  • Work with the CDNM, IDT and relevant stakeholders to improve the services within the Network and lead agency in line with PDS.
  • Maintain links across Networks, the wider Social Work Department and Service as appropriate and as required.

·        The Social Work Team Leader will be expected to maintain and develop advanced knowledge and skills across social work, disability and the development of PDS.

  • Carry a caseload commensurate with duties of a Social Work Team Leader and the needs of the Network. The Social work Team Leader will carry a case load, including a greater complexity of individual/family need, supervise social work staff & students as required and take the social work lead in key areas of service development.
