What is a Family Health Team? In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients. Who is the Hamilton Family Health Team (HFHT)? HFHT is the largest Family Health Team in Ontario. We have over 165 family doctors and over 250 allied health care providers in our organization. We serve over half of the population of Hamilton. There are many opportunities for innovation, leadership, and collaboration with other community health agencies at our public-sector organization. Since starting as chair of our new primary care action team on December 1, Dr. Jane Philpott has hit the ground running. I was pleased to join her and Minister Sylvia Jones today to receive an update on her first couple of weeks and the work ahead to connect everyone in Ontario to primary health care within at least five years.

Last week, leaders from the Hamilton Family Health Team volunteered at the @goodshepherdhamilton Venture Centre - a critical community resource that provides emergency food, clothing, and other essentials to over 170 families every day that their doors are open. ❤️ We were warmly welcomed by the Good Shepherd team and given different tasks, from sorting and organizing clothing donations, to stocking their pantry, replenishing supplies, welcoming guests, and helping individuals and families find items they need, including holiday gifts for their little ones. 🎁🎄 This experience was a salient reminder that a growing number of Hamiltonians are spending the holidays without stable housing, food security, or the essentials they need. While we are partnering with many community organisations and Primary Care Network partners to provide more people in the Greater Hamilton Area with primary health care, there is always more work to be done. We are incredibly grateful to @goodshepherdhamilton for welcoming us and engaging in this important work for our community.
