By 2030, it is projected that around one in four Singaporeans will be aged 65 or above. #NHGP Care Coordinators such as Ms Doreen Chan provide screening and lifestyle advice to help our seniors age well. Find out how Doreen enables patients to maintain their independence and quality of life, in “Up Close With” section in Transforming Primary Care Issue 26: https://lnkd.in/ga7jpJ9v #NHGP #TransformingPrimaryCare

The National Healthcare Group Polyclinics was honoured to be invited by the Agency for Integrated Care (Singapore) (AIC) to attend the launch of REAL, a new community youth mental health campaign by AIC. This initiative, which acknowledges the emotions and challenges faced by youths and young working adults, represents a significant step forward in raising mental health awareness and strengthening community support for the mental health needs of youths. The campaign was officially launched by Dr Janil Puthucheary (Senior Minister of State, Ministry of Digital Development and Information, and Ministry of Health) at the 5th Mental Health Film Festival organised by the Mental Health Film Festival (Singapore) Ltd. Hosted by two dynamic youth leaders from Youth Corps Singapore, the event radiated positive energy and brought together key stakeholders from institutions and social service agencies dedicated to supporting community mental wellness. The audience was deeply moved by the powerful short films created by youths, which addressed sensitive mental health topics with thoughtfulness, authenticity and warmth. These films shed light on the challenges and inner struggles faced by caregivers of individuals with mental health conditions, underscored the vital importance of providing support for those struggling and demonstrated the role of empathy in fostering meaningful connections with individuals navigating mental health challenges. #NHGP was honoured to attend the event as a valued partner, reaffirming our dedication to advancing mental health care, particularly within the primary care setting. It was a privilege to reconnect with esteemed colleagues from Institute of Mental Health and MOH Office for Healthcare Transformation (MOHT), gaining valuable insights from distinguished leaders such as Prof Daniel Fung, Prof Gerald Koh and Dr Weng Mooi Tan. Engaging in meaningful discussions with community partners further strengthened our shared commitment to building a robust mental health ecosystem — especially for youth mental health — by ensuring services are accessible, comprehensive and tailored to the needs of the community. The event left us inspired and energised to continue our efforts in advancing mental wellness. We are deeply grateful for the opportunity to contribute to this meaningful cause and are committed to working hand-in-hand with partners to create a supportive, inclusive and holistic mental health landscape for our community. I had a very fruitful and engaging discussion with National Healthcare Group Polyclinics 2024 graduands and 2025 candidates at the Post-MMed Exam Debrief last evening. Together with #NHGP Drs Darren Seah, Donna Tan, Khai Wei Tan, Gabriel Ding and Wern Ee Tang, we spoke about What's after MMed? in the areas of Clinical Projects, Quality, Contributions to Clinics, Research, and Teaching with both graduands and candidates. I hope the Learning Journey and Exam Experience shared by the 2024 graduands will inspire and encourage our 2025 candidates. I am heartened by the enthusiasm and commitment of all our participants. Together, we will continue to advance the contributions of family medicine to primary care.
