StellarCare NW is a nurse led home care company looking to bridge the gap between health and social care by implementing a more clinical approach to its home care services. Traditionally home care, also known as domiciliary care, provides care and support to people who live in their own home but require additional support with household tasks, personal care and other activities that enables people to maintain their independence and quality of life. Home care is largely seen as independent with an unskilled workforce. Providers of home care services are bound by regulators to uphold national minimum standards for home care however, the industry is often criticised for poor pay and lack of progression leading to large gaps in recruitment to fulfil the required provision of care. Health Care on the other hand is the improvement of health via the prevention, diagnosis, treatment, and other physical and mental impairments in people. Health care consists of professional staff working across primary and secondary care is viewed very differently than home care despite the overlap in care provision when caring for someone at home. From a home care perspective, it is the care workers that regularly see the clients and so, are best placed to report changes and spot signs of deterioration in their clients. StellarCare NW has adopted a nurse led approach and implemented a clinical team leader within its services to meet the changing clinical needs of clients at home. Using live reporting systems to ensure all concerns are raised and reviewed in real time, communications to health care professionals can be made and treatment implemented in short time frames which can prevent disease, reduced hospital admissions and associated costs. The nurse led approach within home care is an integrated model of working that has proved effective in improving client outcomes.
