Announcing the release of a new Population Health Alliance Issue Brief titled "Leveraging Data to Meet Social Care Needs in Support of Achieving Equitable Outcomes."
Social care needs, often called Health-Related Social Needs (HRSNs), include a range of social and economic factors that influence an individual's ability to maintain health. However, identifying who is affected by HRSNs so that effective and efficient interventions can be implemented is challenging due to incomplete, inaccessible, inaccurate, and unreliable data. This issue brief provides key recommendations aligned with the PHA Population Health Management Framework.
