CheckUP are excited to share our new video campaign Healthcare closer to home. Health services are all too often out of reach for Queenslanders living in rural and remote communities. We’re committed to creating healthier communities and reducing health inequities. With the support of approximately 150 contracted health providers, CheckUP’s Outreach Health Services program delivers a wide range of health services throughout rural and remote Queensland, bringing Healthcare closer to home. Health Empowerment Network of Maryland, Inc. a tax-exempt, community-based medical practice offers services that link consumers to comprehensive collaborative prevention and intervention support services to those who are described as senior citizens, low-to-moderate income, and vulnerable populations. The aim of the organization is to eliminate inequality in digital healthcare services and bridge the healthcare technology gap. Through our HNU-One, Health Empowerment Network of Maryland, Inc. takes healthcare to the point of need, bridges the healthcare technology gap and connects healthcare service providers to those who need their help. Additionally, in 2024 Health Empowerment Network of Maryland, Inc. launched its Monthly Health Check initiative. The Monthly Health Checks will primarily focus on increasing health awareness, addressing health disparities, providing prevention/intervention services, and addressing health-related social needs. Each event will target specific health-related initiatives that will decrease health inequalities and increase health equity within the 20743 zip-code region. Community collaboration is imperative, therefore, HENM will partner with civic associations, the faith community, county & state government officials, as well as with other medical providers in our area to ensure success in each event. This year's theme was 'My health, my right,' – emphasizing the right of every person to access healthcare services, information, and education. Despite at least 140 countries recognizing health as a human right, as of 2021, 4.5 billion people worldwide were not covered by essential health services. Addressing Healthcare Disparities in Underserved Communities Healthcare disparities in underserved communities continue to be a significant challenge, with factors such as socioeconomic status, access to care, and cultural beliefs influencing health outcomes. By implementing targeted interventions, embracing and diversifying health education, increasing access to care, and promoting culturally sensitive practices, we can reduce healthcare disparities and ensure that all individuals have access to quality healthcare. Advancing health equity through technology. Healthcare settings are now mandated to screen and report on social drivers of health (SDoH) like food, housing, transportation, financial insecurity, and interpersonal safety. These drivers significantly influence individual and community health outcomes, particularly in under-resourced communities. Closed loop referral systems, like the one at The University of Kansas's Center for Public Partnerships & Research Integrated Referral and Intake System (IRIS), establish critical relationships between healthcare providers screening for SDoHs and community-based partner organizations with the skillsets and community presence to address those needs. As of July 2023, IRIS has made over 50,000 referrals across 32 communities, involving 1,441 organizations, contributing to improved health outcomes for those served.* Streamlined closed-loop processes like the IRIS prevent long wait times, misaligned referrals, and unnecessary burdens on individuals, promoting efficiency and equity. While the concept of communication between healthcare and service organizations is not new, ongoing technology-based efforts, like tracking outcomes through a closed loop referral system, will enhance patient well-being. As part of our efforts to advance health equity through increased community connectedness, KC Health Collaborative seeks to expand and strengthen the network of closed loop referral systems for our entire region to ensure that every BODY has the opportunity to live longer, healthier, more equitable lives.
