Good article about the importance of value based care for the senior population. Almost 80% of seniors have two or more chronic conditions, and seniors typically see seven different doctors between four different practices each year. It is clear that this practice of quality care repeatedly leads to better outcomes.
As leaders from different sectors of the health industry meet in Nevada for Health 2.0 Conference it's an opportune time to talk about ways to drive better health outcomes for seniors. CenterWell Senior Primary Care, a part of Humana is committed to providing better health outcomes for seniors, especially for those experiencing health inequity. In Nevada, 11.3% of seniors live in poverty and 4.7% of seniors have reported avoiding care due to cost. CenterWell Senior Primary Care’s Dr. Tracy J. This is huge: "Healthcare scarcity is not just a problem to be solved; it’s also a lens through which broader societal priorities must be evaluated. Policy makers should recognize that healthcare intersects with almost every other policy domain, from housing (aging in place) to immigration (addressing workforce shortages). The solutions will require cross-sector collaboration, long-term planning, and—most critically—political courage." Healthcare scarcity exists across the continuum of care, and is being experienced by people living in Canada, of all ages. It impacts our experiences of care (as patients, and as healthcare providers), the outcomes of care, and the costs of care. Healthcare should be THE conversation we are having in 2025, in the context of the federal election, and beyond.

The Care Quality Commission health inequalities framework for integrated care systems will be launched in Feb 2025, following the completion of the testing phase. The framework aims to enable health systems to have a positive impact on people and communities experiencing health inequalities. Examples and guidance will be included, as will impact measures. With contributions from National Voices (Jacob Lant), The Point of Care Foundation (Sarah Massie) and others, we look forward to this publication.
