Two nice cases of VT ablation with strikingly similar clinical presentations: hypertrophic cardiomyopathy, similar ECG morphology, and apical origin. The only difference? One was ablated endocardially, the other epicardially! The high-resolution mid-diastolic signals targeted for ablation, recorded using the ablation catheter's microelectrodes, were particularly interesting. These signals allowed for acute termination of the clinical arrhythmia in both cases. DeQuervain's Tenosynovitis New patient seen today for evaluation of 2-year history of chronic right radial styloid pain. Exam and US are consistent with DeQuervain's. US imaging shows significantly enlarged APL and EPB tendons in the affected right side, especially when compared to the uninvolved left side. NOTE: The APL exists in multiple bundles in 80% of normal people! So be careful about making a diagnosis based solely on US imaging.
