What a privilege for my colleagues @MeetaShah and @FelicheBellezza to recently speak at the Health Equity Summit, hosted by Executives’ Club of Chicago. Improving health equity and better addressing social drivers of health are not just the right thing to do – it makes business sense too. Here are a few take-aways from the discussion: Up to 80% or a person’s health is influenced by social factors, such as access to nutritious food or affordable housing, rather than what happens in the doctor’s office. In fact, inequities annually cost the U.S. health care system $450 billion. Accelerating the transition from fee-for-service health to value-based care is crucial, given this approach can help improve access, address social drivers of health, close gaps in care and better support the needs of people with high-risk or chronic conditions. For people with complex health issues, personalized advocacy programs can contribute to more informed decision making and help drive improved coordination across a person’s care team. Leveraging data is becoming increasingly important, both to help connect consumers to relevant community resources and to enable employers to customize benefit plans and clinical programs to better meet the needs of their workforces. Thanks for this opportunity to collaborate. In a period of scarcity of manpower and financial resources, where time for the individual patient is limited due to high registration burden, healthcare cannot keep up with the digital and technological revolution. We might lose our way and each other on the ultimate common goal of healthcare: adding personalised value Healthcare providers, professionals, patients, health insurers and the government have to speak the same language. This report strives to do so ! Keep on doing this together by implementing PCVBHC Maasstad Ziekenhuis , Santeon Genootschap Gepersonaliseerde Zorg Linnean Initiatief Erasmus School of Health Policy & Management (ESHPM)

By bringing together diverse stakeholders like healthcare professionals, community leaders, and policymakers, we can foster open dialogue, share insights, and develop innovative solutions. To ensure flexibility in our timeline, we should consider leveraging digital tools to schedule meetings or dialogues. Additionally, explore the possibility of conducting collaborative sessions in a neutral community setting. These collaborative sessions not only enhance communication but also build trust and strengthen partnerships within the community.
