This year’s Practitioner Perfomance Advice practitioner characteristics Insights paper focuses on Patterns in Practitioner Performance Advice cases with a focus on international medical graduates (IMGs). Key points: · Behaviour/misconduct features in 73% of the cases, similar to our previous periods of analysis. · Ethnic minority, IMGs, older and male practitioners continue to be overrepresented in concerns reported to us by employers and contracting bodies. · IMGs appear overrepresented in GP training grades and there are higher proportions of cases for IMGs who have been in a role for under one year. · NHS Resolution is committed to supporting the fair management and resolution of concerns by working with healthcare organisations through our range of services, sharing learning from our lived experience research and engaging with healthcare partners on EDI initiatives. You’re good at what you’re good at and I’m good at what I’m good at. That’s what I told a medical doctor who contacted me in early December after representing herself using Premium Processing (big no no) to file her own EB-2 NIW as a physician (another big no no) and receiving a whopping 13 page Request for Evidence. Not only did the doctor file as a physician, she never explained her endeavor in any detail. The only point USCIS conceded (in this case USCIS was actually correct) was that she held an advanced degree. We worked dilligently together over the holidays to entirely reframe her case. We collected evidence and expert letters about the past and prospective impact of her medical research and responded on January 4. The approval arrived today-February 22. Lessons: 1) Physician NIW applications in and of themselves cannot likely demonstrate national importance. 2) Detailed specific endeavor statements are a must. 3) Premium Pricessing is probably not a good idea. (Paying thousands extra does not change a priority date and will likely result in an RFE.) 4) USCIS is sometimes right when it issues RFEs. 5) You know what they say . . . the client who represents themselves . . .

WHAT DOES CLINICAL EXCELLENCE LOOK LIKE? Further to yesterday's post we discussed in the 4P weekly group session yesterday also these key points: Developing strong diagnostic and reasoning skills We emphasised the need for clinicians to develop strong diagnostic and clinical reasoning skills. We advised new graduates to be honest about the limits of their knowledge, rather than guessing or making up diagnoses. We encourage seeking mentorship to fill gaps in understanding between the initial and follow-up visits. Building a strong therapeutic alliance We highlighted the critical importance of building a strong therapeutic alliance with clients. We explained that if a patient does not trust the clinician's ability to help them, they are unlikely to engage with the recommended treatment, regardless of its quality. We provided an example of how a breakdown in the therapeutic alliance led to a patient seeking care elsewhere. Addressing contributing factors, not just symptoms We stressed the need for clinicians to address the underlying contributing factors to a patient's condition, rather than just focusing on symptom relief. We provided an example of a new graduate who achieved short-term pain relief but did not address the root causes, leading the patient to seek alternative care.
