CLOVES - an acronym for Congenital Lipomatous Overgrowth, Vascular malformations, Epidermal nevi, Skeletal/Scoliosis/Spinal anomalies - is a rare pediatric syndrome characterized by vascular malformation and soft tissue overgrowth presenting with multiple soft tissue swellings. A recent publication highlights how MSK soft tissue ultrasound can help diagnose this condition. An interesting case of intracranial stenosis in a young patient. Sadly, we are seeing many young patients presenting at a pretty young age with atherosclerotic stenosis. This was one such patient having recurrent TIA with severe intracranial stenosis. In this case, as soon as we placed the guiding catheter, we saw clot formation beyond the stenosis. Initial doubt was whether it was an air bubble, but clearly, it kept increasing in size and we confirmed that it was a clot formation. In fact, guiding catheter placement and a slight change in hemodynamics triggered platelet aggregation. One can explain this clot formation based on turbulent flow beyond the stenosis, oscillatory and low shear that can cause platelet aggregation and thromboembolic strokes. We lysed the clot using injectable antiplatelet agents, and thereafter we did angioplasty followed by stenting with a self-expanding stent, ultimately achieving good hemodynamic reconstitution. The side branches were preserved. We did dilatation with a drug-eluting balloon to decrease the chance of recurrent stenosis. This paper reports a case of an 88-year-old hypertensive female patient with sustained atrial fibrillation and recurrent #heart_failure who was finally diagnosed as the stage 3B cardiac amyloidosis secondary to lambda light chain myeloma, highlighting the importance of an early diagnosis of cardiac light chain amyloidosis. #Monoclonal_Gammopathy
