Lumbosacral fracture with vertebral dislocation and pneumothorax. Referred to NVA clinic the following week for surgical therapy. ➡ X-ray diagnosis: Lumbosacral dislocation and fracture of the sacral arch in S1 with involvement of the sacral vertebral canal by the body of L7, dorsally dislocated. ➡ Surgery: Reduction and stabilization. Meet Mookie! Mookie is a 2-year-old French Bulldog who was transferred to our hospital from his regular veterinarian. Mookie had developed bladder stones that needed to be removed immediately. They had created an obstruction in his bladder and this was preventing him from being able to urinate. Mookie was prepped for a surgery called a cystotomy. This procedure involves making an incision in the bladder to remove, in Mookie’s case, the stones creating the obstruction. During surgery, several small stones were removed from Mookie’s urethra. After surgery, Mookie began urinating normally and was hospitalized on supportive care until his pain was well controlled. Once he was feeling better, he was able to go home! Mookie’s stones were sent for testing and came back as cystine type stones. Cystine is a type of amino acid in the body that is normally reabsorbed by the kidneys. Cystinuria in an inherited condition that occurs when the kidneys are not able to properly reabsorb cystine, leading to the formation of stones. Left untreated, a urinary obstruction in any animal will be fatal. We prescribed a urinary diet for Mookie, which can help dissolve and prevent any further stone formation events. The patellar tendon angle (PTA) is the angle subtended between the axis of the tibia and the patellar tendon and is positively correlated as an intrinsic risk factor of anterior cruciate ligament (ACL) rupture in veterinary patients. Therefore, one of the surgical treatments used in animal knees is the tibial tubercle advancement (TTA), which reduces the PTA and, thus, ACL stress. So why not propose this surgery for humans as well? Today, I am pleased to share this new study conducted at the Centre Lyon Ortho Clinic, Clinique de la Sauvegarde, where we compared the PTA of a group of patients with ACL rupture and a control group, and we found that the PTA were not elevated in the study group. What’s the relationship with the patellar tendon vector and the ACL??? The patellar tendon angle changes with knee flexion… as the knee is extended, the patellar tendon angle increases, resulting in a larger anterior shear component of quadriceps force acting on the tibia and a larger ACL strain. Vets modify this angle through surgery on the tibial tuberosity as one treatment option for dogs with Cranial Cruciate ligament rupture (synonymous with the ACL in humans). #zoobiquity By increasing knee flexion during landing, this vector, along with ACL strain, can be reduced. In this paper we failed to demonstrate anatomical differences in those with ACL rupture compared to controls, suggesting the best way to modify this vector in humans is through modifying knee flexion with functional activities, not with surgery.
