For an NHS that meets the needs of today's society:
Care needs to be integrated and community-focused (this means investment in primary care and rethinking new models of care).
We need capital investment (we cannot continue with outdated equipment and buildings in disarray).
We need investment in digital infrastructure
And finally, we have to tackle drivers of poor health. And this will require policies and initiatives from within and outside of the NHS

To ask you, first of all, you're on the front line obviously, of the National Health Service. Just give us a flavour of what it's like on a daily basis where you work. Yeah, so I, I'm a GP and, and yeah, it's difficult. And I think, you know, we've seen in the NHS the situation get worse and worse year on year. And in general practice, I guess we're at the, you know, we're at the sort of the war edge of it. And we are seeing, you know, our workload has been increasing. We have patients who are left months, sometimes years on waiting list. They keep coming back to us because their conditions are getting worse, their pains getting worse. And throughout the NHS is suffering with staff shortages and. And I think, you know, mobile, it's quite low amongst NHS workers. Burnout is high in general practice. Amongst my colleagues, I know that recent polls found that three out of five GP's were planning to leave the profession within the next 5 years. And across the country, you know, GP practices are closing, there's about 100 practices closing every year. So the, the situation, the NHS is not good. It's not good for people working in the NHS. It's not good for patients either. I think you know the findings of the report, I think anyone that's worked in the NHS. Will you know, will be quite familiar with what was discussed in that report and and I think you know, it feels. It feels as if it just works. You know, we have been talking about this for quite a long time now, the worsening conditions in the NHS and, and I guess it feels every time this comes up, we hear from governments that they will make changes. They want reforms. And I think we are now really sort of waiting now for those reforms to come in place. I don't think we have 10 years to wait. These reforms need to be coming in sooner than that. And Case Starmer said, hasn't he, that there's no funding without reform. And he's talked about things like, you know, transitioning the VHS to digital. They're moving more care from hospitals to communities, focusing efforts on prevention over sickness. Do those elements, do you think, sound like common sense to you? Or do you think that there are other parts of reform that are more urgent? They absolutely sounds like common sense and I, I but, but, but money is needed, funding is needed. I think if you look at, if you look at go, just go to your local hospital, you'll see that there is out of date equipment, some buildings in disarray and some GP practices, they're using sort of really out of date equipment. And so to, to ensure that we have a modern NHS into digitalize our NHS that will need money. And I think it's very frustrating, frustrating for those of us working in NHS when you have sort of different, different systems and. Can IT systems not working well together? So we absolutely need to digitalize our NHS. And 2nd to that, bringing more care into the community is really, really important. You know, we've been saying that as health workers and as GPS for a long time, but also patients want that. They want to have their care in the community and they're absolutely agree with the prevention. We're talking about prevention. And I know I think this doesn't get talked about enough. You know, get yes, we always talk about waiting lists and how situations getting worse in the hospitals. I think we don't talk about it. Up is that the health of our nation is also been getting worse, so we've been seeing more and more people being getting mental health conditions at a younger age. So we absolutely need to be seeing more action from our government to tackle that. And we also need to be open that it's not just the NHS that we'll be able to do that. We need to be thinking about policies outside of the NHS to improve the health of our nation and absolutely has to be a priority for this government has been really good to talk to you and I'm grateful to you for taking the time to speak to us. 💰💰to reduce the elective backlog
I’m sure more will follow, however I’m equally sure that more fundamental changes are required if we are to protect the "the lifeblood of Britain" as Chancellor Rachel Reeves called the NHS.
We need to work hard to:
📈Improve productivity- by global standards, the NHS isn’t bad, but we can do better
👫Address the challenges surrounding workforce, and make it easier for those working in the NHS
🗺️ Make it easier for patients to navigate the NHS
🏠Provide the right care, closer to home
☎️📲Accelerate the move from analogue to digital

Launched at last week’s NHS Providers’ annual conference, this report explores the opportunities and challenges of group working within the NHS and offers guidance on how trusts can develop group models that help them to balance responsibility “at-scale and at-site”.
Christian Dingwall, Partner in our Health and Life Sciences team, and Janet Budd, Director of the NHS Transformation Unit, offer their thoughts on the report in our article below.

Lord Darzi's report provides concise clarity on the challenges facing the NHS, evident to many for so long. It’s refreshing to read a report which very much aligns with our thinking. Achieving long-term sustainability, care quality, and patient satisfaction requires a fundamental shift of focus toward delivering the right care in the right setting. Maximising the potential for community-based facilities to deliver more and higher acuity care is an absolute must.
That being said, and as the report recognises, years of capital starvation mean there is an urgent requirement for major investment in acute facilities. This would fix fundamental problems, like leaky roofs and malfunctioning sewage systems. But, through evidence-led design and incorporation of new technologies, it also has the potential to unlock huge amounts of productivity gains and improvements in care.
As important as finding the money for this investment is transforming the structure that determines how the money is spent. Yes reform of CDEL and the business case process are a must, but so is de-centralisation of capital allocation decisions to regional and Trust level.
