BRAVE Southeast Asia Tech Podcast: Yudara Kularathne: D*ck Pics for Science, Child Emergency Doctor to AI Founder & Model Training vs. Regulatory Affairs - E523 Yudara Kularathne MD, FAMS(EM), CEO & Cofounder of HeHealth, and Jeremy Au 区汉辉 discussed: 1. D*ck Pics for Science: Yudara described the progress of HeHealth, an AI-powered men’s health app launched in 2022. The app helps users detect sexually transmitted infections (STIs) by analyzing images. Within two weeks of its release, the app received over 20,000 downloads globally, including from countries like Uzbekistan and Kazakhstan. Yudara recounted a case where a user with monkeypox contributed 150 detailed images documenting his condition over two weeks to help improve the app’s diagnostic accuracy. This approach leveraged community contributions to create robust, annotated datasets while adhering to ethical guidelines. 2. Child Emergency Doctor to AI Founder: Yudara reflected on his journey from managing over 10,000 cases as a pediatric emergency doctor to founding HeHealth. During his time at Sengkang General Hospital, he built a dedicated pediatric emergency department with support from KK Women’s and Children’s Hospital. The COVID-19 pandemic disrupted hospital workflows and inspired him to address healthcare gaps. He cited a personal encounter in 2019, when a friend mistakenly believed he had HIV and considered suicide, as the moment that fueled his commitment to bridging stigma-driven healthcare disparities through technology. 3. Model Training vs. Regulatory Affairs: Yudara emphasized the balance between building AI models and navigating regulations. HeHealth used incremental data collection and synthetic data to improve accuracy and reduce biases, particularly for underrepresented groups. In 2023, HeHealth faced a $50 million FTC lawsuit in the U.S., citing privacy concerns about sensitive user data. Using AI tools for legal strategy and transparent communication, Yudara successfully resolved the case within two months. He highlighted how innovation often outpaces regulation and the importance of proactive collaboration with regulators to mitigate risks. Jeremy and Yudara also talked about the role of synthetic data in addressing racial biases in AI healthcare tools and the challenges of scaling health tech in emerging markets. Coming home to Singhealth Research in 2025 to share my HeHealth journey as a Startup Founder. As public sector clinicians, we were skilled in many areas, but the startup journey proved to be a completely different challenge that required learning an entirely new set of skills. The transition was not easy, but I’m happy to share every step of my journey. If you’re considering taking your clinical career in a new direction, this might provide some helpful insights. During the session, I’ll discuss how I built a startup leveraging disruptive technology in a traditional field, how I developed data and intellectual property through community engagement, and how I established a globally recognized research team and achieved impactful research outcomes—and much more. Does Primary Care Need a Digital Health Revolution? - Here’s My Take Primary care is the backbone of healthcare systems. It’s where patients first turn to for help, yet it’s often most under-resourced & overwhelmed part of the system. In my experience working in digital health research, I’ve noticed that while healthtech is booming, primary care lags in adoption. Why I believe this gap exists—and how to address it: 1️⃣ Fragmented Workflows Primary care clinics often rely on outdated systems that don’t communicate with each other, leading to inefficiencies and burnout. 50-60% of providers experience burnout, with family physicians reaching 90% - primarily due to administrative burdens and fragmented systems (JAMA, AHA). 💭 What if we designed digital tools that integrate seamlessly into the daily workflows of primary care providers, instead of adding to their workload? 👉🏼 Focus on interoperable solutions that connect patient records, diagnostic tools, and communication systems in real time. 2️⃣ Providers Don’t Feel Supported Many providers feel overwhelmed by the complexity of digital tools. 70% of providers report challenges with digital tools, yet 80% believe AI could help (JMIR, WHO). 💭 Are we investing enough in equipping providers with the skills and confidence to use digital health tools effectively? 👉🏼 Develop intuitive platforms and provide ongoing training for primary care teams to boost both adoption and confidence. 3️⃣ Patient Engagement Barriers Digital tools often focus on providers & neglect patients. If patients can’t engage with these tools easily, the impact is limited. Barriers like literacy and tech access limit engagement (BMJ Open). 💭 Are we designing tools that truly empower patients to take charge of their health, or are we leaving them behind? 👉🏼 Create tools with patient-friendly interfaces and features that promote health literacy, like reminders, self-monitoring, and easy access to care plans. Imagine a Primary Care System Where: • Digital tools reduce provider burnout by automating repetitive tasks and streamlining workflows. • Patients actively participate in their care through intuitive apps and platforms. • Providers and patients alike feel empowered by technology, not overwhelmed by it. This isn’t just about innovation—it’s rethinking how we deliver care at the most fundamental level. I’m glad to be part of AIGP Health, working toward similar mission with smarter, digital solutions. What’s Your Take? - Are we doing enough to innovate primary care, or is there more work ahead? #DigitalHealth #PrimaryCare #HealthcareInnovation (Image generated by ChatGPT-4)

When you are creative and innovative, technology works magic. What do a chimp, a hippo, a leopard, an elephant, and a cheetah have in common? Not much—except that the Apple Watch works perfectly on all of them to measure their vitals, just like it does for us. Of course, it's clearly not FDA-cleared for use in animals, but if it's better than the next best available technology, why not? In sexual health, there hadn’t been significant innovations for years—until HeHealth disrupted the field by creating groundbreaking use cases with existing technology. Yet, some people couldn’t accept it. My message to the world: if something is significantly better than the current baseline for most people and has a larger impact, don’t crucify the innovators. Instead, open your eyes and see the potential. My takeaway from 2024: Open your eyes, open your heart, and be flexible enough to embrace new things. The world is changing. AI vs Doctors: AI Wins by Margins. In scenarios where doctors score 30% and AI scores 80%, AI emerges as the clear winner by significant margins. As a patient, would you consider using AI for a second opinion? In the study referenced, researchers compared the performance of O1 Preview with physicians on NEJM case records across multiple aspects, including differential diagnosis. Diagnosis, often regarded as one of the most challenging tasks in medicine that physicians take pride in, seems to be an area where O1 preview demonstrates superior reasoning. The margins are so significant that even with the largest possible margin of error, AI still outperforms. On the other hand, since AI is trained on virtually everything available on the internet, it is possible that some cases may have been part of its training data. However, this doesn't negate the results. The direction is clear—we need to prepare for this shift. As more studies will emerge in 2025, we’ll see additional comparisons. With O1 already fully operational and the more advanced O3 on the horizon, it will soon be nearly impossible for doctors to outperform AI. We need to adapt to working alongside AI. In the near future, not utilizing AI in medicine could be considered negligent. While humans may be the weaker link in terms of raw accuracy, we should focus on providing care with empathy and emotional connection—an area where we excel compared to machines. 😊 #ai #medicine #future #diagnosis
