We were overwhelmed by the positive feedback from trainees to community psychiatrists on our pilot program results at the American Psychiatric Association Annual Meeting at 2024! A pilot outpatient care model funded by NYC Department of Health and Mental Hygiene and NYC Office of the Mayor, Hamilton-Madison House's CONNECT program is a community outreach focused mental health program, primarily geared towards underserved populations in downtown New York City who frequently fall into access gaps (housing, benefits, justice system involvement, debilitating physical health etc). We present initial results pooled from August 2022 to February 2024 that patients enrolled in our program stay in treatment, use emergency room and crisis calls less, and show appreciation for flexibility and continuity from the treatment team to meet their mental health goals! Thank you to co-authors Janice Ng, MPA, Tse-Chien Chen, Jiayue Mao, Fanny Kuang, LMHC for contributing to research, current/past team members Danielle Baum, Alice Tsao, Jeff Huang, Jamie Marrara, Esther Li, Paige Kim for the outstanding work on (literally!) the ground, and Mike Zhou & Lilya Berns, MS, OTRL for program supervision that allows our work to continue!
