Just presented my poster at HitRi 2024 - 8th Congress of Emergency Medicine! Our case report, "Pulmonary Thromboembolism Concealed Behind Lumbar Pain," sheds light on a critical diagnostic challenge. Our patient, a 36-year-old female, presented with severe lumbar pain, which upon investigation, unveiled a pulmonary thrombus. This atypical presentation underscores the importance of recognizing diverse symptoms of PTE for prompt diagnosis and treatment. A 40-year-old CP female presented to the Emergency Department complaining of increase shortness of breath and respiratory distress. Past medical history was unknown. Physical examination: revealed blood pressure of 140/90 mmHg, heart rate of 70 bpm, respiratory rate of 22 per minute, O2 sat of 66% on room air, and temperature of 37°C. There were internal jugular vein distention & regular heart sounds EKG: confirmed right atrial enlargement (pulmonale P) and right ventricle hypertrophy with right axis deviation. There were RBBB and RVH pattern,inverted precordial T waves. CXR: prominent pulmonary artery and some congestion with cardiomegaly. Echo: showed high SPAP, RVH , RV & RA enlargement Laboratory data were remarkable for : Hb : 19.8 with low serum iron and ferritin K: 5.6 , Cr: 1 and other normal data. Treated with IV furosemide as dieuretic and oxygen therapy to stabilize. Low dose and slow infusion serum therapy after decongestion. Phlebotomy could not be done because of low ferritin to treat polycitemia. Referred to heart center for RHC and other subspecialty procedures to find the cause of pulmonary HTN.
