🧐 When it comes to writing your health condition copy, you might be missing one of your biggest sources of content. How often do you ask patients and families after they received care what they wish they had known? Especially when it comes to serious conditions, at some point we all have to dig deep to become armchair experts for ourselves or our loved ones. Does your copy answer... ✍️ What it's really like to get a procedure or refuse treatment? ✍️ What to expect during recovery ✍️ What are typical and rare side effects of a therapy? Your copy doesn't just exist so people can find and choose your health system. Done well, it can be a part of patients' care journey. So many hospitals have patient and family advisory councils (PFAC). Consider asking them about your copy. You'll no doubt learn something. H/t to Sara McKinniss for inspiring this post.

Building our brains trust here today at The Department of Health and Aged Care. When we try to solve problems on our own, we are limited by our own knowledge, experiences and beliefs. When we open up to others and ask for their support the sky is the limit. You don’t need to have all the answers. You need to have the courage to ask for help and invite in new ways of thinking. #facilitator #TeamBuilding #TeamPlanningDays

Hey #PEI - Did you know that you can become a patient and family partner with Health PEI? Check out Wendy's testimonial and learn more about how you can make a difference.
"Since first being introduced to being a Patient and Family Partner, attending meetings and having great conversations with people that actually listen and have the ability to make changes has been invaluable to me. Having first-hand experience with the MH&A services I am aware that there is work to do to improve some aspects of the services offered. Knowing my voice and ideas are being heard makes a huge difference. Some of my ideas and suggestions have been put into play proving that my voice is truly being heard. Sitting back complaining about a service is one thing, but having your voice heard to help improve it…. Is an amazing feeling!" -- Wendy MacDonald, Health PEI Patient and Family Partner Become a Patient and Family Partner: https://lnkd.in/eNEHkDMX

Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.
The recently released evaluation of Project Roomkey showed what is possible when there is political will, cross-sector coordination and collaboration, and person-centered approaches to housing and health care. In this new California Health Care Foundation blog, you can read about two Alameda County Project Roomkey 'sheroes' - Alexis Chettiar and Catherine Hayes. These amazing clinicians leveraged their Roomkey success and learnings to create, in collaboration with Five Keys Schools and Programs and other key partners, transitional and permanent housing for medically frail people. They 'do what it takes' for their patients and are an amazing model that we should be replicating across the state. Read about it below. #housingishealth #CalAIM #crosssectorcollaboration

𝗧𝗵𝗲 𝗶𝗺𝗽𝗼𝗿𝘁𝗮𝗻𝗰𝗲 𝗼𝗳 𝗸𝗶𝗻𝗱𝗻𝗲𝘀𝘀 𝗶𝗻 𝗿𝗲𝗵𝗮𝗯𝗶𝗹𝗶𝘁𝗮𝘁𝗶𝗼𝗻 🌱 Kindness is often overlooked in clinical settings, but its role in rehabilitation is profound. On World Kindness Day, we highlight why kindness should be a key consideration in patient care: • Emotional resilience: Kindness reduces stress and builds trust, creating a safe environment for recovery. • Motivation: Small acts of kindness, like words of encouragement, can empower patients to push through challenges. • Stronger connections: A kind, empathetic approach strengthens relationships between healthcare professionals and patients, leading to better outcomes. In the context of rehabilitation, kindness is not just a value—it’s a powerful tool that complements clinical expertise. Let’s continue to integrate kindness into every stage of care, ensuring patients feel seen, valued, and supported 🤎 #AskhamRehab #Rehab #Neurorehab #WorldKindnessDay #RehabilitationJourney #KindnessMatters
