Availability of the right equipment during a case makes life easy for the doctor and the patient. During a primary PCI on RCA , I was faced with a long segment calcific RCA disease that was easily dealt with using a 6F LiquID guide extension with a 7F ID.. This made passage of a 4.0 stent a breeze. Also demonstrated is the inch-worm technique(and a little bit of distal anchor) to coax the guide extension over a balloon when a simple push wont do! kudos to the LiquID team for a flexible and usable catheter! #primaryPCI, #accessmedical, #LiquID, #cardiology, #burjeelhospital

Yesterday’s #ClinicalChallenge question was: True or false? In patients with severe aortic stenosis, long-term mortality rates are higher with transcatheter aortic valve implantation (TAVI) compared with surgical aortic valve replacement (SAVR). The answer is: False. In a recent trial in 280 patients with severe aortic stenosis who were randomly assigned to receive a bioprosthetic valve via transcatheter implantation or surgical replacement, mortality rates at 10 years were similar between the two groups. Case Study: PCI for LAD Occlusion in a 72-Year-Old Male performed in Innvolution Cathlab. A 72-year-old male presented with an Anterior Wall Myocardial Infarction (AWMI) and underwent thrombolysis. Despite this treatment, the patient continued to experience angina. The following day, angiography revealed a 100% occlusion in the Left Anterior Descending (LAD) artery, prompting a plan for Percutaneous Transluminal Coronary Angioplasty (PTCA) to the LAD. During pre-dilation, the balloon did not expand due to an eccentric calcium nodule, so a cutting balloon was used instead. Post-dilation angiography showed dissection, and the patient’s blood pressure dropped. The doctor promptly deployed a stent, followed by post-dilation with the balloon, successfully sealing the dissection. The final result was excellent, and the patient remained stable.
