#WFITN2024 #INReporters 👉 𝗣𝗿. 𝗚𝗼𝗯𝗶𝗻 talked about the various options of 𝙨𝙪𝙧𝙜𝙞𝙘𝙖𝙡 𝙩𝙧𝙚𝙖𝙩𝙢𝙚𝙣𝙩 𝙛𝙤𝙧 𝙞𝙣𝙩𝙧𝙖𝙘𝙧𝙖𝙣𝙞𝙖𝙡 𝙞𝙙𝙞𝙤𝙥𝙖𝙩𝙝𝙞𝙘 𝙝𝙮𝙥𝙚𝙧𝙩𝙚𝙣𝙨𝙞𝙤𝙣 𝙄𝙄𝙃. To resume: ➡️ optic nerve fenestration is obsolete, moderately effective & on visual signs only. It does not address other symptoms of IIH. This technique has been widely used but the drawback is a high rate of minor complications & recurrences. ➡️ CSF shunt : effective on all symptoms & fast but the main limitation is the recurrence at 12 months of symptoms around 40%. ➡️ Bariatric surgery is a “physiologic” treatment (BMI more than 35) effective if weight loss of more than 24%. The rate of complications is significative at the post op period & long term. ➡️ Venous stenting appears to be the most appropriate “physiologic” intervention, most effective on symptoms with a recurrence rate at 12 months between 10-15%. Rare but severe brain hemorrhages exist and related to the lack of flexibility, trackability of the stents. Pr. Gobin highlighted the fact that using telescopic multiple stents is a risk factor of having a brain hemorrhage. Using a single long stent could increase the safety of the procedure & limit the complications. 👉 The 2nd speaker highlighted the fact that we are still lacking strong data to guide our decisions and recent metanalysis is showing that only 1/3 of patient benefit for durable clinical benefit. 💡𝐒𝐭𝐚𝐲 𝐭𝐮𝐧𝐞𝐝 𝘋𝘳. Some of the common post operative complications are: 𝐏𝐨𝐬𝐭 𝐨𝐩𝐞𝐫𝐚𝐭𝐢𝐯𝐞 𝐛𝐥𝐞𝐞𝐝𝐢𝐧𝐠: Major Postoperative Bleeding (mPOB) is the most common complication after bariatric surgery. It can range from self-limiting to life-threatening situations. 𝐍𝐮𝐭𝐫𝐢𝐭𝐢𝐨𝐧𝐚𝐥 𝐝𝐞𝐟𝐢𝐜𝐢𝐞𝐧𝐜𝐢𝐞𝐬: Nutritional deficiencies are a crucial consideration after bariatric surgery. Bariatric procedures can affect Vitamin B12 absorption. 𝐏𝐨𝐬𝐭 𝐨𝐩𝐞𝐫𝐚𝐭𝐢𝐯𝐞 𝐋𝐞𝐚𝐤𝐬: This complication occurs when there is a leak at the surgical connection site (anastomosis). It usually happens within the first few days after surgery and rarely after 2 weeks. 𝐔𝐥𝐜𝐞𝐫𝐬: Ulcers may develop in the stomach or small intestine after bariatric surgery. These can cause pain, bleeding, and other issues. Gallstone Formation: Rapid weight loss after bariatric surgery can increase the risk of gallstone formation. 𝐓𝐡𝐫𝐨𝐦𝐛𝐨𝐞𝐦𝐛𝐨𝐥𝐢𝐜 𝐃𝐢𝐬𝐞𝐚𝐬𝐞: Although rare, blood clots (thrombi) can form in the legs (deep vein thrombosis) or travel to the lungs (pulmonary embolism) after surgery. Hence, reinforcement of staple line gets extremely crucial to avoid these complications.
