The OneNeurology initiative aims to unite and strengthen neurology-related groups to stimulate collaborative advocacy, action and accountability for the prevention, treatment and management of neurological disorders worldwide. Under the OneNeurology Partnership, international neurological organisations and regional umbrellas are uniting to make neurology a global public health priority. It is a multi-stakeholder, patient-focused partnership which aims to strengthen neurology-related groups to stimulate collaborative advocacy, action and accountability for the prevention, treatment and management of neurological disorders worldwide. OneNeurology's founding partners are the European Federation of Neurological Associations (EFNA) and the European Academy of Neurology (EAN). Other partners are Alzheimer Disease International, the World Stroke Organization, World Sleep Society, European Brain Council, World Federation for NeuroRehabiliation, Women's Brain Project, European Peadiatric Neurology Society, International Neuropalliative Care Society, Confederation of Meningitis Organisations and the IHS-Global Patient Advocacy Coalition. 🌟 We are pleased to announce our latest publication in the Nature Magazine - Nature Aging 🌟 Informant characteristics are associated with the Clinical Dementia Rating Sum of Boxes scores in the Alzheimer's Disease patients participating in the National Alzheimer’s Coordinating Center Uniform Data Set "The clinical dementia rating scale-sum of boxes (CDR-SB) is the primary endpoint of all clinical trials for Alzheimer's disease. In this study we found that informant characteristics have significant effects on the CDR-SB and may impact all clinical trials for Alzheimer's disease." Dr Juan-Camilo Vargas-González - Author The Clinical Dementia Rating® Sum of Boxes (CDR®-SB) is a globally used assessment tool for measuring cognitive and functional impairment, applied to stage dementia severity in Alzheimer’s disease clinical trials. What if the scores actually changed based on the caregiver's gender or their relationship to the patient? Indeed, it appears that when the caregiver is female, CDR®-SB scores tend to indicate higher levels of dementia severity compared to scores observed with male caregivers. Given that the CDR®-SB is designed to assess the patient’s cognitive impairment, this finding highlights a significant bias in a widely utilized tool for staging dementia in clinical trials. Here, the authors assess whether caregiver-related factors, the caregiver’s sex, but also the relationship to the patient, and frequency of contact, might influence CDR®-SB results. The analysis includes data from 20,636 participants with mild cognitive impairment or dementia, drawn from the National Alzheimer’s Coordinating Center Uniform Data Set (NACC-UDS). This study is the first to demonstrate the impact of informant characteristics on CDR®-SB outcomes. Some of the key findings of the study are: 👉 The CDR®-SB scores were higher when the informant was a female compared to men 👉 The CDR®-SB scores were higher when the informant was the patient’s child compared to spouses or partners 👉 The CDR®-SB scores were higher when the informant lived with the patient or visited daily, compared to when visits occurred at least once a week It is critical to commit to further investigation of these associations to ensure the robustness of the CDR®-SB against such factors and eliminate bias from non-disease factors. Investigation should focus on examining the potential effects of additional informant variables on the CDR®-SB and explore strategies for controlling or adjusting the score as needed. Today is World Mental Health Day! I’m pleased to be in Geneva this week, attending meetings organized by the WHO Department of Mental Health, Brain Health, and Substance Use. On Tuesday, I represented the European Federation of Neurological Associations (EFNA) at a meeting of relevant non-state actors officially affiliated with WHO. It feels like this meeting will really move the needle in deepening opportunities for civil society in this space. Yesterday and today, I’m at the WHO Mental Health Forum. This Forum is particularly special, as the last in-person gathering took place in Geneva five years ago. I vividly remember attending it and witnessing how, for the first time, WHO began conceptualizing what “brain health” means. Today, I was invited to sit on a panel representing the global partnership OneNeurology, which EFNA founded a few years ago. We discussed the implementation of the global action plan on neurology. On the topic of mental health, WHO Director Tedros Ghebreyesus noted that mental health remains one of the most neglected areas in health. Many member states took the floor to share their comments. The US reiterated President Biden’s statement that mental health care is healthcare. Guyana spoke about poor mental health being an existential threat that is destroying lives. Until we view mental health as we do cancer, diabetes, or pandemics, we will not be able to fully integrate it into our policies. The Ukrainian representative emphasized the need for timely interventions before, during, and after crises to mitigate harm. Others highlighted the need for trauma-focused mental health care centred on recovery. The US and Chile have begun working on a resolution regarding social connection, which will be discussed at next year’s World Health Assembly. I’ve witnessed many WHO meetings where member states address various health topics, but I can clearly sense that there is a different tone and level of engagement when they discuss mental health. During the Forum, the Lancet Psychiatry Commission on Mental Health in Ukraine was launched: https://lnkd.in/gQRY3wrU It is encouraging to witness the incredible progress made in recent years in neurological conditions, mental health, and brain health in general. I feel a deep sense of fulfilment working in this important field of public health. One way I’m trying to take care of my well-being while attending full-day meetings is by choosing accommodation far enough from the venue to allow for a solid walk. Today, I celebrated World Mental Health Day with a one-hour walk. On my way, I saw a beautiful chestnut tree and picked up a chestnut from the ground. It brought back fond childhood memories. Happy Mental Health Day!
