A new model of delivering community services in York is set to be considered by the council's Executive later this month. Councillors will be asked to consider plans to reshape early intervention and prevention services around four new integrated neighbourhood teams. If approved, the new neighbourhood teams would bring together professionals from different organisations across health, care and community services, housing, parks and open spaces, to build community capacity. 🎁 Day 2 of our 12 Days of Christmas campaign Today we're sharing how City of York Council tackled pension poverty by using the Low Income Family Tracker (LIFT) platform to identify eligible residents and maximise their income. The initiative enabled cross team collaboration, reaching hundreds of households and improving financial security for pensioners. 💬 Pauline Stuchfield, Director of Housing and Communities said: “This Pension Credit Campaign has helped combat rising pensioner poverty and adult social care costs by bringing money to the pockets of pensioners across the area.
