Am excited to share that our editorial article titled 'Emergency Medical Services Impact Evaluation Approaches in Low and Middle Income Countries', that I've written alongside Dr William Stassen of Cape Town University and Dr Sabrinah Ariane Christie of University of California Los Angeles (UCLA) has been accepted for publication in the journal Surgery by Elsevier! In the article we advocate strongly for the need to decolonise the way we think about M&E in the 'development' sector. We ask: 1. Does it really make sense to think of Western country ambulance and emergency response systems as models to be replicated in countries that have very different socio-econo-cultural contexts? 2. Does it make sense that 'high income' countries, many that colonised and continue to exploit their former colonies, should be leading the conversation about what counts as 'impact' in those countries? 3. How can we reimagine, in partnership with the communities themselves, how to assess the impact of EMS systems? After the article comes out next month, I will be blogging about how these ideas apply to EMS systems in Pakistan like Saving 9's Embrace Mental Health and Maseeha Ambulance systems

Is blood pressure (BP) variability linked to cognitive decline in older patients? A recent study published in the journal of Neurology hints people with the most variation in BP often face slight cognitive decline. There is evidence to support treatment of hypertension reduces risk associated with slowed thinking ability. The data researchers used came from the Chicago Health and Aging Project. Love this. My fave book about this from the last few years is Midlife: A Philosophical Guide by Kieran Setiya. Recently wrote about Setiya because he helped me consider: what do you do after you realize your profession’s dream? Setiya says that this question occurs for most of us in midlife. When you reach midlife, he observes, you look back at your travels, surveying the paths you took, the paths you did not take, and the paths you ruined. This can be distressing, as you realize that fewer choices are available to you at midlife. Some options have permanently expired, and there is no objective way to tell if you traveled the best path. And you can find yourself thinking unanswerable questions. Physicians find themselves asking questions like: -was cardiology really the right field for me? -should I have skipped med school entirely? -what would it have been like to be a poet? -to have followed Virgil instead of Hippocrates? When asking these midlife questions, you can become bored by past accomplishments but feel unable to set out toward radically new ones. Setiya’s advice is to seek the value you receive from being immersed in the activity of daily life. He calls it the atelic life instead of the telic life. The paradox of a medical career is that medical training is about pursuing accomplishments, the hero work, the telic life but the actual practice of medicine is about caring for the sick, the atelic life. When you care for patients day in and day out, the value comes from being immersed, day after day, in the care of the people you meet as patients. It’s not heroic, like in medical dramas, but endurance in the daily acts, much like the parenting and partnering.

WOHW Day 3 Research and evidence-based practices There are more and more osteopathic professionals pursuing academic degrees and involvement in research in the last ten years. Students are growing into research and evidence-based practice as a vital part of their education as in all healthcare studies. This prepares the students into inter-professional working and research. In the United States osteopathic physicians are easily involved in the academic research due to the nature of their education. Adolescent medicine, also known as adolescent and young adult medicine, is a medical subspecialty that focuses on care of patients who are in the adolescent period of development. This period begins at puberty and lasts until growth has stopped at which time adulthood begins. The psychosocial period of adolescence is extended both by an earlier start, as the onset of puberty begins earlier, and a later end, as patients require more years of education or training before they reach economic independence from the parents.
